Please click one of the appointment links below and fill out all the required information to schedule your COVID-19 vaccination. Please do not show-up more than 15 minutes early to your scheduled appointment, or you will be asked to wait in your car.
Once you schedule your appointment, you are guaranteed a vaccine. If you cannot self-register on-line, or a family member or friend cannot assist you in registering, you can call the Johnston Senior Center at (401) 944-3343 and ask for assistance.
We anticipate receiving all COVID-19 vaccines through the Rhode Island Department of Health and all appointments are on a first come first serve basis.

We are limited to 250 doses per week, spread out over four weeks. The Moderna Vaccine will be administered and you can find addition information regarding this vaccine here:https://www.modernatx.com/covid19vaccine-eua/
All vaccination appointments are being conducted at the Johnston Indoor Recreation Center located at 1741 Atwood Avenue between the hours of 10:00 a.m. and 2:00 p.m. on recurring Wednesdays and Saturdays
